We pay the full cost of Basic Life Insurance and Basic Accidental Death & Dismemberment (AD&D) Insurance. Both coverages are equal to two and one-half times your base pay (2.5x), up to $500,000 in coverage.
You can buy employee supplemental term life coverage for yourself, your spouse or domestic partner, and your dependent children. You must enroll for employee supplemental life and AD&D insurance coverage to be eligible to enroll for spouse/domestic partner and/or child supplemental dependent term life and AD&D.
Life and AD&D Insurance Options at a Glance
- Basic Life Insurance – 2.5 times annual base pay*, up to $500,000 in coverage
- Basic AD&D Insurance – 2.5 times annual base pay*, up to $500,000 million in coverage
Optional Life Insurance Coverage (100% employee paid)
- Supplemental Employee Life – Up to an additional $500,000 in $10,000 increments. .
- Spouse/domestic partner Supplemental Life – up to $250,000 in $10,000 increments
- Supplemental Child Life – $2,500, $5,000, $7,500 or $10,000. Children are eligible from live birth until age 26 (if a full time student)
Optional AD&D Coverage (100% employee paid)
Employee: Increments of $10,000 to the lesser of 3x your annual base salary or $500,000 – (not to exceed
10 times Earnings for amounts over $150,000)
Spouse / Domestic Partner / Child(ren):
- Spouse with no child(ren): 50% of the
employee amount - Spouse with child(ren): 40% of employee amount
- Child(ren) with spouse: 10% of employee amount
- Child(ren) with no spouse: 15% of the
employee amount
Don’t forget to update and/or review your beneficiaries
You must designate beneficiaries for your Basic Life and AD&D Insurance and your employee supplemental term life and AD&D Insurance, if applicable. You are the beneficiary of any AD&D benefits payable for accidental injury. You are also the beneficiary of your spouse or domestic partner and children’s Supplemental Life and AD&D benefits in the event of their death.
